A 28-year-old male presents with arthralgia, fever and malaise which has been ongoing for 2 weeks

A 28-year-old male presents with arthralgia, fever and malaise which has been ongoing for 2 weeks. He is normally fit and well. He does not take any regular medications and has no allergies. He smokes 10 cigarettes a day and drinks 20 units of alcohol a week. Five months ago he went to Thailand where he had vaginal and receptive oral sex with a sex worker. He has no other travel history.
