A 34-year-old man goes to his GP complaining of night sweats and vague pain in his abdomen and chest. The GP does a thorough history and examination. On examination he is found to have an early diastolic murmur and splinter haemorrhages on his hand. He describes being unwell for some time, with no other problems apart from a recent tooth infection and abscess, which required drainage and eventual removal of 2 teeth. He denies any history of rheumatic heart disease, valve problems or IVDU.
