A 36-year-old male complains of a lump in his right testicle

A 36-year-old male complains of a lump in his right testicle. He is unsure how long it has been there and does not cause him any pain. He feels well in himself but is concerned as he and his wife have been struggling to conceive, despite trying for the last year. An urgent ultrasound is booked, bloods are taken for tumour markers, and an urgent referral to secondary care is made. His alpha-fetoprotein (AFP) comes back normal.
