A 37-year-old man is found on the street by the paramedics unresponsive and drowsy. They notice that he has poor capillary refill, is unable to stand and also notice increased pigmentation on the creases of his palms. His basic observations show a pulse of 110, lying blood pressure of 70/50mmHg, temperature of 36.5 degrees C and an oxygen saturation of 95%. They immediately give him some buccal glucose gel, start him on intravenous fluids, and take him to the emergency department.
