A 42-year-old woman presents to A&E, having fallen from her horse (2)

A 42-year-old woman presents to A&E, having fallen from her horse. She hit her head, followed by a loss of consciousness for around a minute. Whilst she was in an ambulance she was increasingly drowsy, and on assessment in A&E her GCS is 12/15. She has a CT scan, which is reported as showing an acute subdural haematoma, and on her way back from the scanner she loses consciousness.

What is the emergency surgical intervention that would be indicated in this case?
