A 5-week-old baby girl is brought to her General Practitioner (GP) on the advice of a Health Visitor who noted that she is small for her gestational age. She was born at 32 weeks. On examination, the infant is in the 2nd percentile for her corrected age and looks tachypnoeic. On auscultation the GP notes a continuous murmur which he describes as ‘machine-like’.
