A 55-year-old male presented to his GP with occasional episodes of haematuria and gradual onset of breathlessness over the past six months. However, he denies any other urinary symptoms. Physical examination was unremarkable. Urine dipstick reveals blood ++ but is negative for leukocytes and nitrites. In view of his breathlessness, his GP decided to arrange a chest X-ray which showed multiple cannonball lesions in both lung fields.
