A 56-year-old woman presents to the GP with a 6-month history of a chronic cough which is productive of green sputum. She has been treated with antibiotics for chest infections six times in the last year, one of which required a hospital admission. She denies any weight loss or night sweats. She has a past medical history of rheumatoid arthritis, which is treated with once-weekly methotrexate. She has never smoked. On examination, she has marked finger clubbing and coarse inspiratory crepitations over the left lung base.
