A 58-year-old lady presents with sudden onset copious productive cough and dyspnoea. Symptoms began rapidly and seemed to have been exaggerated. There is an audible “squeak” with every breath and the patient looks extremely unwell. She denies any chest pain, night sweats and haemoptysis. She is currently on methotrexate for rheumatoid arthritis and takes this once a week. Examination reveals widespread inspiratory crackles.
