A 58-year-old man presents to the Emergency Department (ED) with increasing shortness of breath. On examination he has a raised JVP and crackles can be heard at both lung bases. He has extensive peripheral oedema. On palpation of the radial pulse, it is noted that the pulse is regular but seems to alternate between strong and weak. The consultant tells you that this sign is indicative of severe left-sided heart failure.
