A 58-year old man presents with a six-month history of difficulty swallowing

A 58-year old man presents with a six-month history of difficulty swallowing. He describes a history of progressive dysphagia in which it feels like food is ‘getting stuck’ in his throat. He first managed this by cutting his food into smaller amounts, but now even the smallest portions of food are difficult to swallow, and he is also having problems swallowing liquids such as soup and yoghurt. He has lost around two stone in weight over the past six months, and has had a poor appetite. He has had problems with acid reflux for many years, for which he takes daily omeprazole. He drinks around 30 units a week of alcohol.
