A 59-year-old man has had a left-sided spermatocele for the past two yearr

A 59-year-old man has had a left-sided spermatocele for the past two years. He has always found it painful and had the fluid aspirated from it a year ago. Unfortunately, it has increased in size again, so he visits his GP to discuss management options. His past medical history includes moderate COPD, heart failure, AF and metformin controlled type 2 diabetes mellitus. He has advanced bilateral OA of the knees but is ineligible for replacement due to his significant comorbidities. He has 3 children in their 30’s and is not looking to have any more at his age.
