A 6-year old boy presents with bilateral pain in his hands and feet

A 6-year old boy presents with bilateral pain in his hands and feet. He has had recurrent attacks of pain for around three months, and has recently been unwell with several chest infections. A full blood count shows low haemoglobin, a normal mean corpuscular volume, and a raised reticulocyte count. Neither of his parents have suffered from anything like this, and he has older two siblings, who are both fit and well.
