A 60-year-old man presents to his GP complaining of itchy skin

A 60-year-old man presents to his GP complaining of itchy skin, which has been worsening for 2 months. On closer questioning he is also feeling more bloated than usual and has a dull ache in the epigastric region, as well as a constant, non-positional dull ache in his middle back. He does not report any weight loss but feels “tired all the time”. On examination there is 3 finger breadths hepatomegaly.
