A 62-year-old male COPD patient presents for annual review at his general practice. He is currently being treated with salmeterol (a long-acting beta agonist). He complains that he is still struggling to keep pace with his wife when they go out for walks together, due to his breathlessness, and finds that he still wheezes and coughs a lot on exertion. He has never required hospital admission and has not required use of rescue packs.
