A 62-year-old presents to A&E. She tells you that a few hours ago she lost the sight in her left eye

A 62-year-old presents to A&E. She tells you that a few hours ago she lost the sight in her left eye, and it felt “like a black curtain being drawn” over her sight, to which turning a bright light on and trying to turn her head to see around the “black-out” made no difference. It then eased off after a few minutes, and her sight is now back to normal.

A CT head is done, with no obvious abnormality. The patient is started on high dose aspirin for treatment of a TIA, and carotid dopplers subsequently show moderate bilateral stenosis in the left and right common carotid arteries. The patient is started on clopidogrel long-term. An ECG shows a new diagnosis of atrial fibrillation, and the patient is also diagnosed with stage 1 hypertension.
