A 67-year-old man with a history of hypercholesterolaemia presents to the GP

A 67-year-old man with a history of hypercholesterolaemia presents to the GP because he has been feeling tired all of the time for the last two months.  You notice in his records that he has had multiple throat infections over a similar period. When you ask, he also admits that he has had some back pain over the last 6 weeks which is not improving, but he put this down to doing too much gardening.

Cardiac, respiratory and abdominal examinations are unremarkable, and a digital rectal examination is normal.  His observations are:

  • Heart rate 82
  • Respiratory rate 14
  • Blood pressure 131/89
  • Oxygen saturation 98%
  • Temperature 36.7°C

You decide to carry out some blood tests.  His investigation findings are:

  Baseline Current
Haemoglobin (g/L) 140 113
WBC (x109/L)  6 5
Sodium (mmol/L) 141 143
Potassium (mmol/L) 3.6 3.9
