A 70-year-old attends A&E complaining of visual loss

A 70-year-old attends A&E complaining of visual loss. Earlier that day he suddenly lost the sight in his right eye, with no associated pain or any other abnormality – he tells you his vision “just went black”, and that it was definitely only that eye, as he tested both. After around 5 minutes, the vision gradually returned and he now feels back to normal. A CT head is done, with no obvious abnormality.  The patient is started on high dose aspirin for treatment of a TIA, along with medications for secondary prevention. Carotid dopplers subsequently show moderate stenosis of the right common carotid and severe stenosis of the left common carotid arteries.
