A 72-year-old man is brought to A&E by his neighbours

A 72-year-old man is brought to A&E by his neighbours. He was found wandering a nearby street in a state of undress and did not recognise his neighbours, who they say have known him for 25 years. He is unable to tell you where he is, or what day it is, but appears to be quite calm. On further questioning, he tells you a long-winded story about being asked to attend a meeting by your secretary, and that he did not want to be late for his appointment. His neighbours say he has lived alone since his wife died five years ago. You refer to his GP records, which note a history of alcohol excess. He was last seen by his GP two years ago. You did not note any evidence of alcohol intoxication when reviewing him.
