A middle-aged man comes to see you as he has noticed a lump on one of his testicles. He is unsure how long it has been present for as it causes no pain; he only noticed it recently. On examination, there is a small palpable lump in the epididymis, separate to the testis. It is non-tender, soft and fluctuant. He has no associated symptoms and generally feels well in himself.
