A tall, 32-year-old man presents with acute onset shortness of breath and chest pain

A tall, 32-year-old man with Marfan syndrome presents with acute onset shortness of breath and chest pain. He recalls that this pain came on when he was running this morning. The chest pain is central, with no radiation. An arterial blood gas (on room air) shows: Hb: 120 g/L; pH: 7.49; pCO2: 3.2 kPa; pO2: 16.0 kPa; HCO3: 24 mmol/L.
