An 81-year-old lady is admitted with confusion and is found to have deranged liver function tests. An abdominal ultrasound shows several areas of increased echogenicity that look suspicious. A CT abdo pelvis reveals what looks like malignant metastasis. She is too frail for any form of treatment, and so a palliative approach is taken. She is unable to cope at home, and so a new placement with social services is found. However, in the time it takes to source this, she deteriorates to the point that she is too unwell to be transferred. Her oral intake is non-existent; she develops nausea and vomiting and is in significant distress, so a syringe driver is started with analgesia and an antiemetic.
