An 82-year-old lady with severe dementia is admitted and treated for a urinary tract infection. You are called to see her because she has become unresponsive. She usually smiles and nods, but now her gaze appears fixed and she doesn’t respond to you when you are talking to her.
On examination she is not following commands. She is laying very still, and her eyes are looking forward. She does not follow you with her eyes when you move, and her blink-to-visual-threat response is absent on the left-hand side.
