A 22-year-old male presents to A&E 2 hours after waking with severe right scrotal pain

A 22-year-old male presents to A&E 2 hours after waking with severe right scrotal pain. He also has lower abdominal pain and nausea. He is tachycardic and hypertensive; all other observations are normal. He has a past medical history of appendicitis which was treated conservatively. On examination, the right scrotum is erythematous, oedematous and tender. The testicle has an abnormal lie, and elevation of the scrotum does not relieve the pain, but the cremasteric reflex is intact. His urine dip is negative.
