A 29-year-old female presents to the emergency department with chest pain and shortness of breath. She has no cough or fever. There is no significant past medical or family history, though she had recently arrived via a long-haul flight from South America, where she had had bilateral breast augmentation surgery five days previously. She neither smokes nor drinks alcohol. Her observations demonstrate tachycardia and tachypnoea. Of note on examination, she has an erythematous, swollen right calf.
