A 42 year old male presents with sudden onset vertigo that has been continuous since around 10am. He is worried he is having a stroke and has had trouble keeping his balance. On questioning, the vertigo is not worsened or triggered by head movements. He has had no features of tinnitus or noticeable hearing loss and this is backed up by a bedside cranial nerves examination which also reveals mild horizontal nystagmus. He says he has felt “a bit off” recently with a sore throat and mild cough troubling him around a week ago.
