A 5-year-old child presents with right ear pain and right hearing loss that started yesterday. She has had a cold for the last three days with a runny nose and fever. She has not had previous episodes of ear pain. On examination of the right ear with an otoscope, you see a red and bulging tympanic membrane and a fluid level.
