A 52-year-old woman is being treated in an inpatient psychiatric unit for an acute relapse of schizophrenia and is currently being treated with high dose aripiprazole. An emergency call is put out, as she has become suddenly confused and agitated. On assessment, she is febrile, tachycardic and tachypnoeic, and disorientated. She is complaining of severe pain in her arms and legs, which are rigid on examination. An urgent blood gas shows a metabolic acidosis.
