A 6-year-old boy presents to the emergency department with increasing shortness of breath

A 6-year-old boy presents to the emergency department with increasing shortness of breath that started this morning. Since yesterday, he has had a hoarse voice, a cough and has not been eating as he finds it difficult and painful to swallow. On examination, there are white-grey exudates in his oropharynx. He is making a high pitched, inspiratory wheezing nose but his chest is clear. He also has tender and enlarged cervical lymph nodes and a fever. He has been well previous to this episode. He has recently emigrated from Bangladesh, and his vaccination history is unclear.
