A 79-year-old man presents with 4 months of pain in both legs. The pain is constant, worse on leg elevation, and relieved by dangling his legs off the bed. His right leg has become excruciatingly painful over the past day. 

He has metformin-controlled type 2 diabetes mellitus. He is an ex-smoker with a 50 pack-year history.
