Mrs Burke, a 67-year-old lady, presents with an itchy, sore right nipple. Over the last week the itching has become more intense, and yesterday she noticed some bloody discharge from the nipple.  

On examination there is an eczematous lesion of the nipple with areas of ulceration. There is also evidence of nipple discharge.
