Mr Jones brings his 2-year-old daughter Jenny into the GP practice. She has been irritable all night and complaining of ear pain on the left side. Mr Jones noticed some discharge on Jenny’s pillow this morning. Mr Jones had noticed a fever and had given some paracetamol. Jenny is eating and drinking well. Jenny is known to have no medical conditions, take no regular medications and has no known drug allergies.
On examination, Jenny looks miserable. She has a temperature of 37.8, HR 129 (95-149), RR 23 (20-30) and saturations of 98%. On otoscopy, the right tympanic membrane is normal. The left tympanic membrane is red, swollen and perforated with a greeny-yellow mucinous discharge present.
A diagnosis of acute otitis media is made, and the GP provides Jenny with a prescription of amoxicillin oral solution (125mg/5ml) with a dose of 250mg 3 times a day for 7 days.
