You are a junior doctor working in general practice. A mother presents with her five-week-old baby concerned she is consistently vomiting milky liquid after most feeds. Her daughter has also been quite irritable and difficult to settle. She has previously brought up some milk after feeds but this volume has increased over the last few days. When asked, the mother says the vomiting is not particularly forceful. She is currently bottle-fed.
On inspection, she is alert, pink and warm. All other observations are currently normal. She weighs 4.3kg.
