Investigations: given his claudication and the chronic ulcer, a duplex ultrasound of the lower limb arteries or CT angiography to assess for significant peripheral arterial disease affecting healing.

The duplex shows significant left superficial femoral artery stenosis. How does this change your plan?

This confirms peripheral arterial disease, with poor perfusion delaying wound healing, so I would treat the perfusion and the wound together.
Discuss revascularisation options such as angioplasty or bypass surgery to restore blood flow, which is essential for the ulcer to heal.
Start optimal wound care: debridement, appropriate dressings, and negative pressure wound therapy if indicated.
Refer to the diabetes team for glycaemic control and to podiatry for specialist foot care and footwear.

Two weeks after revascularisation he develops a wound infection. How do you manage it?

Prompt recognition and treatment are critical.
Assess the extent clinically, and image if osteomyelitis is suspected.
Start empirical broad-spectrum antibiotics, then tailor to culture and sensitivity results.
Surgical debridement if there is necrotic tissue, with infectious diseases input for complex cases.
Close wound monitoring, frequent dressing changes, and ongoing coordination with the multidisciplinary team.
